I don't mean stuff like secret menus where you can get things that are unlisted but are still made up of normal components...
I mean stuff like when you can get steak and spaghetti... at a McDonalds.

Like an episode of Columbo I saw recently where the detective orders chili at a fancy French restaurant. He gets it, and they charge him $25, which would be a lot for chili now, but this episode was from the 1970s, so that would be more like $150 today.
When I worked at restaurants I got creative all the time. Ordering from one, other than basic things like no pickles or something I think it's an unnecessary imposition on both foh and boh staff workers who by and large aren't paid enough to put up with the assembly line let alone requests going beyond said line.
