They almost always come down to a combination of unfamiliarity, miscommunication, and (most importantly) being rushed by superiors.
But yeah they really don't happen very often. Like in my work area it would be like 2 per 15,000. But when it's a small package it's much more likely to be missorted because multiples of them grouped together in nylon bags, so when one of those bags goes to the wrong place they all do. The bagging process is also automated so it actually ends up making more mistakes than humans. It'll occasionally throw the wrong stuff into a bag even if the bag label tells the loader it's all good to go.
