What option do you pick at the gas station

Regular.

What's even the difference anyway?

Higher octane gas resists pre ignition, or knocking. So if a car says to use a higher octane than regular, it's a good idea for the engine's health to do so.

Conversely, putting a higher octane than recommended offers little if any benefit, besides maybe more detergents.
