I never played MQ, but Forest Temple had this one irritating key in the normal mode that's a little outside the entrance to the dungeon. It's still technically the dungeon since when you transition there the first time, the name comes up, but it's actually in the woods and OUTSIDE of the building. Took me an hour or so to find.
