These are 2.2 million people would have died if we didnt do what were doing, Trump said at the briefing, during which he cited that number 16 times. And now were looking at numbers that are going to be much, much, much lower than that. And it makes everything were doing feel much better to me.

A raise of equivalent value would be $4000+/year for a family with 2 children. No company is going to give such a raise. THey're going to offer an insurance plan that covers it for much than $4000+/year.

Dental / Orthodontic costs aren't just about a simple exam and cleaning every 6 months.

Still not an entirely free market, but something closer to it is dental care. People sometimes buy dental insurance, it can be a good idea, but it's not mandatory nor is it completely tied up with employment. You can also just pay out of pocket and unlike other healthcare, the costs are sensible.
