Often it's a combination of depression and poor life circumstances that causes someone to retreat into food. By the time they realize they've gone too far, they can't stop their cravings and they can barely move let alone work out. At that weight, doing things like getting up and sitting down is exhausting and because people think of exercise as an endurance effort, people just up before even trying because they can't even stand well. To make things worse, once they fall into this pit there's usually at least one family member that's enabling them without really realizing it. Someone had to be cooking for and cleaning this woman, and whoever did it likely made things worse by just buying and cooking whatever she wanted.
