Normally both thumbs are going the same direction - i.e. both thumbs are going away from the body at the top of the rotation and towards the body at the bottom of the rotation. Or both are going toward the body at the top and away from the body at the bottom.

This is one martial arts technique that I have yet to master, along with patting with one hand while rubbing in circles with the other. I guess I can kind of do it very slowly.
