In character creation it always winds up looking wrong, somehow.

The developers don't care.

There was something in - IIRC - CNET about an independent group of black & Latino developers making open source code to more accurately reflect the skin tones that actually exist in the world.

Their goal is to get other developers to use it. We'll see.

How so? Any examples?

I can name one, in FFXIV it's very hard to make a good looking Midlander Female that has dark skin, I think it can be called "wrong" or "off", it just looks weird.

But with Viera it looks much better, skin tones aren't the same between the races and I never understood why.

Because they just use a color slider, but that's not how skin works in reality. It's not even how lighter skin tones works, but it's not as noticeable. They'd need to address it more like they do hair, with multiple colors for undertones and highlights stuff.

This seems like something that could be easily made. It's just not

They know most of the people demanding on Twitter don't really care and will move onto to the next thing of the week once they make the barest token effort

I've also noticed sliders that desaturate any color hue when it gets to a certain darkness gradient. The only reason I can see for this is to save memory by constraining the color palette in-game. So instead of having a very dark blue (a cool skin tone) or dark red hue (a warm skin tone), like with normal dark skin, you end up with a very dark grey.

Most don't realize that dark skin does not apply to both hand palms and feet soles due to those specific parts lacking melanin.
