If you see some Bullets with red capes don't attack them they will fly away after like 10 seconds. Do that 5 times and you unlock Bullet.

Does it have to be one run and when do they start appearing? Never seen one or does it start after beating the game?

Can be done on multiple runs. They start appearing as soon as you beat one past.
