When do you decide an employee's conduct needs to be reported to management?

As a customer what's something you consider a severe enough in your interaction with an employee that you won't tolerate it and expect some kind of consequence to be issued for the way you were treated?

We all know the frontline grunts put up with a lot of shit and reporting them over every little slight can make someone's life a lot worse than it already is but there are definitely limits.
