When do you decide an employee's conduct needs to be reported to management?

I have never encountered a case where I felt I needed to talk to a manager. But I also don't tend to get that pissed off about something. I would rather just make a mental note to never go back to that store or avoid that person next time I am in a store rather than tattle on them to management.
I feel like I need to put something here, or else I am one of those weird people who think that having no signature is a character trait.
