Well since stuff like trolling and flaming are subjective, and I pretty much call Pokewars an idiot on a daily basis and never get modded, I'll say for the cut-and-dry stuff, yeah maybe I get hit 75% of the time.

Depends on the board. Trolling gets modded way more often on active game boards than social or special interest boards.

It really just depends on how many people mark things. I go to a few boards and there a things that happen on all of them that are/were technically against to ToU and other board rules, but unless there is like a sticky or something a lot of stuff remains untouched.
