Twice.

1916 - Woodrow Wilson (New Jersey)
1844 - James K. Polk (Tennessee)

Twice in 56 presidential elections. This election, Mitt Romney will very likely lose his home state of Massachusetts. Based on these historical numbers, losing a home state does a candidate no favors at all, so he's got a tough road ahead.
