It depends. In a left<->right on the political spectrum sort of way it would be the middle point in extreme anarchy (far left) and extreme authority (far right). In that one you are correct: it is about policy preferences. In a pop culture sense it just means someone that is extremely far-right, but doesn't like the label of "Republican." In that one you are wrong: it's not really about a particular position so much as a desire to pontificate about how extremely smart they obviously are because they have no actual practiced stances that they are willing to disclose.
