That was about sending you back after you turned the system off. Sending you back after you died or got game over was more due to lengthening how long the game took you, especially once they started trying to fight game rentals. Ninja Gaiden 1 and 2 were semi-reasonable in that you had unlimited continues, so at worst you got sent back to the start of an act. Ninja Gaiden 3 (at least in North America) had limited continues and no way to get more, so once you ran out of those you were back at the start of the game.
True, but you just went back because once you turned your system off, your progress was lost. Once you could save your game, you started to see longer play, with a larger focus on narrative. That happens to be my preference, so I'm fine with it. There's probably plenty of difficult indie games. There's also a handful games with a permadeath mode.
