I think I hew closer to "real person," while the fandoms tend to treat them the same as they would any fictional character, and it's been like a decade and I still haven't figured out a way to process this. I guess it really depends on how much the person behind it leans into that fictional character. It is difficult to keep the act up for a long time though, usually they let out their real personalities and hobbies and that would become part of the character as well.
