Gabriel Knight II: The Beast Within, which has six CDs. Also, the original Baldur's Gate. Five CDs for the base game, and the sleeve has a sixth slot for the expansion disc.

Some of the floppy-based games could be nuts, but I don't own them any more. Gabriel Knight 1 had 11 floppies. Time Zone had twelve if you count each side separately, and that was in 1982. Amiga games could have even more floppies than that, but that's because most Amiga users didn't have hard drives, and consequently the data on the floppies needed more redundancy and less compression.
