Sega originally had plans of releasing two versions of the Saturn, one discbased and one cartridgebased.
They skipped the cartridgebased one when they realized how costly those cartridges would be to be if they wanted the same storage size as a disc.
That RAM cartridge slot was never meant to have any game cartridges.

Yes, it was intended for extra RAM since day one, it was a nice way to keep the console price down to have extra RAM as an addon. Cheating devices like Gameshark and Pro Action Replay used the slot so technically it should work. You might be forced to start the console with a disc in order to make it start, and then have the cartridge highjack the data and load itself into the RAM instead.
I got very little knowledge of the Saturn hardware, but since cheating devices works, I cant really see any reason why it wouldnt work.
