I was the best man at my brother's wedding.

I started with a standard cheesy canned joke: "I'm told that in giving a best man's speech I'm supposed to sing my brother's praises and tell everybody what an awesome person he is. Unfortunately I'm a bad liar and a terrible singer."

Then I made a couple jokes about how we used to fight when we were kids. Like actual fighting, throwing punches and full-on submission holds. There was a slideshow on loop in the corner of the reception hall that I referenced and told people to check out because it had a picture of me literally sitting on top of him with his leg over my shoulder.

Then I said once we grew up and he went to college, I realized what an awesome brother he was and how we got really close. He'd always been there for me no matter what, and he'd never let me down. I mentioned how, sophomore year of college, when I brought a few girls from my dorm over to his house for a party I never imagined I was introducing him to the only person who would form a bond with him that would rival our own.

Used that the segue into talking about his wife, saying how beautiful and intelligent she is and all that. Then finally wishing them good luck and hoping for a life of happiness and fulfillment together.

I then spent the entire rest of the evening getting showered with praise from the entire crowd for giving the best speech of the night. ^_^

Basically, here's the steps:
1. Start with a generic joke. Use the same one I did, if you want. Best man speeches are the one time nobody cares if it's cliche.
2. Poke fun at the groom.
3. Praise the groom.
4. Mention the first time you saw the bride & groom together.
5. Praise the bride.
6. Wish them well, and toast.
