It is my pharmacist who will be like, "Oh, you are taking ________ right now? Make sure not to take it within 2 hours of the meds the doctor prescribed. It can cause some bad interactions" The computer doesn't differentiate between what is and isn't clinically relevant. Just today, I took a call from a PA because he was admittedly paranoid about ordering citalopram and ondansetron for his patient because the computer flagged it. I guessed that he was concerned about QT prolongation, and he was. Cardiac history? None. Is that ondansetron every 8 hours as needed? Yep.
