Not 100% sure and it's kinda hard to pick. Few contenders, though.

I liked S-Cry-Ed's opening, not simply for "Reckless Fire" (which is a simple catchy tune) but the opening video which, among other things, gradually changes as the show progresses to reflect characters who have died, etc. (Although it's not entirely clear that one actually died, because Cougar shows up after his supposed death.)

I also like Lucky Star's intro (which has a lot of fun parodies), Tokko's opening, Hellsing's opening, and Love Hina's opening (mostly for the opening notes).
