First Lady isn't a government post at all and "Advisor" is more a vanity position than anything, although it's closer to being an actual government post. The First Lady traditionally doesn't even have a security clearance (I'm not even sure Hillary had one when she was First Lady, but Melania certainly doesn't). Neither is comparable to actual government posts that require either a vote or an approval process.

It's bad that you don't understand the difference between a Secretary of State with top secret classified information who needs approval to be appointed to her post and has extensive government clearances because she's one of the four most important people in an administration vs a First Lady with no security clearances, no classified intel, etc.
