That was done by someone that had access to the winning tickets inside the marketing company, he also had to use multiple people outside of the company to get part of the winnings. Meanwhile some random dude at a Taco Bell store isn't guaranteed to get the winning ticket even if he takes 100 cups and uses all the codes.

He also would have to explain when customers complain constantly about not being able to use the codes if the cups were replaced. You act like the guy that pulled off the monopoly game fraud over years was just a random burger flipper at a McDonalds. When the prizes scale into the $1 million range they usually have far more security for the winning ticket. I also will point out he was eventually caught, he may have gotten away with it for a period of time but he didn't get other people to redeem all these prizes, and they never figured it out.
