Does this come across as arrogant?

I went to a mechanic today to have my car's brakes replaced and to get an oil change. While I was there, one of the employees greeted me with "Hi, my name is Mr. Walsh."

I thought that was strange because nobody ever introduces themselves by their last name. I thought it was a joke and never referred to him by his name. But I looked at his name tag, and it said store manager, and his last name was indeed Walsh.

After I was done waiting in the lobby, they call me up for payment. One of the newer employees was apparently inputting my order and says "Mr. Walsh, how do I do this?" Like seriously? Introducing yourself to customers like that is one thing, but having your employees call you the same thing? I thought this might have been done in the 1960s, or am I missing something here? Who does this?
