Or was he just so massive at the start that losing the initial weight wasn't good enough and he still needs to lose a bunch more? iirc he got a gastric sleeve which resulted in a lot of initial weight loss, but failed to address his habits/psychology around food so he found a way to bypass the sleeve and still regain most of the lost weight. And now he's yoyo'ing back down again. For his sake I hope it sticks.
