I'm going to guess a mix. Most US people are pill poppers for one thing or another, so pills are very easy things to let into your body. So the initial bias to seek information that confirms your fears (of injections) isn't there at the jump. I think for most anti vaxxers, at least SOMEBODY they know has died, been hospitalized, or dealt with relatively long term respiratory issues to want to avoid that experience, but their fear of the vaccine and the imagined side effects/ conspiracies overrides that. As we have seen, in most cases being and anti vax person doesn't stop then from taking up ICU space, so they are not completely anti medicine (aside from a few stand outs).

Then there are the people that have well and truly bought into naturalist bs and alternative medicine. Maybe they'll fall out of it down the line when they realize it's all bs, but they'll avoid the pill while it's relevant.

This is all out of my ass speculation though.

People aren't anti-vaxx because they're afraid of needles.
