Always?
What if they have a mental illness?
How severe does it have to be?
What if they choose to not take the medication to help control that mental illness?
What if they just had a large event like the death of a loved one happen which consumes them in negative emotions?
What if someone is forcing them to do something?
What is force though, a gun to your head is one thing but what if there is no threat of violence?
What if someone is emotionally manipulating them?
What if it's an order from a higher authority?
What about their age, if they're still young?
What about if they have low intelligence?

Etc. Etc. I'm not asking anyone to answer these specific questions, they're just some food for thought to help answer the question in the title.
