Because you are essentially letting your 5 year old dictate everything.

No you're not, you're just giving them the freedom to dress the way they'd like to. Bathrooms are a point of contention, and I'd probably try to explain to them that they should continue to use the boy's bathroom in public settings, but otherwise, letting your child tell you what they'd like to be called and how they'd like to dress really isn't too radical a concept. Adopted kids often get to choose a new name for themselves, and even non-adopted kids might ask their parents to stop calling them by another name they decide sounds childish. It's not that big of a deal.

Adding to and nurturing the confusion is tantamount to child abuse.

Acknowledging a child's feelings is not abuse, but ignoring them and forcing them to behave as a gender they don't identify as is pretty irresponsible parenting. If they change their mind at age 6, so what? They're a kid, I wouldn't expect them to know who they are yet either way. There's no harm in giving them a bit of freedom to express themselves as long as they aren't hurting anyone else.
