Isekai series can have all sorts of MCs. They don't have to be incredibly cliche! There's a western fantasy series call The Chronicles of Thomas Covenant the Unbeliever where the MC has leprosy and doesn't believe the world he gets transferred to is real, and he ends up ~~raping the first woman he comes across~~.
