That has nothing to do with Survival mode, that's just a normal random event. Basically, there are a number of places in the game that trigger random events, and each one can trigger specific events. One of the first ones you'll encounter is a campfire site between Red Rocket and Concord. Depending on what the RNG decides, you can find nothing there, or some raiders, or hostile animals, or a traveling doctor. http://fallout.fandom.com/wiki/Fallout_4_random_encounters You can actually look on the wiki and see specific events or people and which areas they trigger in, and then save scum to get the specific event you want to occur.
