Target the actual risk/disadvantage/etc (whichever is applicable) factor, rather than using some other trait as a loosely-accurate (if even that) proxy. They target homosexual men, or more specifically men who have had sex with other men, because they tend to have a wildly disproportionate HIV infection rate. In the US, for example, gay men might make up 4% of the male population, but almost 70% of all HIV infections. (Please allow for some wiggle room for men who have had sex with other men, but don't identify as gay).
