The World of Darkness setting which is a rival to DnD, is set in modern day earth but with stuff like vampires and werewolves. And one of their game lines is Changling The Dreaming. You play as a Faerie Folk basically. You look like a human to normal people but to other Fae, you look like your true form whatever it may be. Like some look like Goblins I think. Or elves.
