a comforter is just a slightly fancier blanket, to be used with a top sheet.

a duvet is a gigantic blanket that's used without a top sheet and a duvet dover is pretty much a pillowcase the duvet goes in, but typically it's used more for decoration

however, in the US, a duvet is called a comforter despite not being one
