Refuse the promotion. Why accept all the additional responsibilities and work with no recompense pay?

Why accept a promotion without a raise? That's virtually them telling you that you are responsible for more work now at the same pay.

does your promotion involve doing more work/more responsibilities or did they just upgrade your title to match what you do

Tell them you appreciate the promotion and recognition, and you look forward to the new role, but you would also appreciate increased compensation to go along with it. Or else you're prepared to walk, or go back to the previous role.

Should work.

I just recently told my boss to not put me as charge nurse because she refused to give me a raise because I had previously gotten a raise when I became charge at another clinic with same company. No back to regular nurse but far less responsibilities and same pay

Now you have the higher job title, apply for that job with another company. Your own company will match the salary if they don't want to lose you.

Even though the market for employees is tightening companies still don't want to pay them more, it would cut into profits

Dont say anything since it likely is a lose lose situation. But immediately put your resume out there and feel free to field calls from potential employers at work (well, go outside etc to be discreet but yeah). Tough it for now in the meantime and at least have the knowledge youre gaining experience in your current position.

If you know they need you... meaning you are irreplaceable.

Then I would definitely say something or threaten to leave.... in a polite manner lol.

If you are talking about a job where any derp off the street can do it, then keep your mouth shut.
