As I've learned from some specials on TV over the last 20 years, many American restaurants (and this varies by region) consider ketchup to be an abomination. They'll serve it, but it's not seen as a staple. For example if you were to go to a place that serves hotdogs in Chicago you'll see bottles of ketchup on the tables and counters, but never on the dogs themselves. Mustard, relish and mayo are seen as superior condiments. Then you have some places that mix mayo and ketchup together to make fry-sauce. Now where I am in Canada if you go to McDonald's like any other McD's around the world if eating inside you'll likely see a ketchup dispenser.
