Usually in western version X is ok and O is cancel, and in Japanese version O is ok and X is cancel

It's been that way since the SNES: A was the default ok button and B was the default cancel in most Japanese games.

For the O and X thing it's probably because they use an O for something when it's correct and the X is wrong. So having it as you accept something with O and don't accept with X makes sense.

The real question is why though, couldn't they just leave it the same and save a crap ton of money by not making several variants.

But why did they ever bother to change it for the US then? And why have the button placement like it is. Having to press the button on the right for accept just feels wrong.

it's not that they switch it, but it switched during localized/international version

why?

because in their culture, X means no and O mean yes

but, when the western game come to japan, they didn't remap the buttons

so, why they decided to make X as an "okay" button? AFAIK, there is no real reason, but maybe many country that use latin/adjad writing system use X as some "yes" marking decisions

O = OK and X = No: circle the option you want, and cross out the ones you don't. Makes sense.

X = OK and O = No: put an X on the checkbox for the option you want... and put a zero (0) value on the ones you don't want? Maybe.

Y was usually used as the menu button, whereas they tended to move it to X for the US version.

Anyways, it's just the way things originally were and so that's how they remained. My guess is that the people who did localization for Nintendo of America felt that using B as the ok button felt more natural and thus localized most games that way.

The split in control schemes started back then and have probably remained that way out of tradition. I'm sure most older Japanese gamers would feel that X being ok and O being cancel feels just as weird to them as the reverse is to us, just because that's what they're used to.
