In this case it would be by gun or sword. To have the duel, you would reserve a prepared dueling ground and have an armed judge, in case someone cheats. You pay a fee, reserve a time slot, and sign a waiver saying that you understand any and all consequences.

It's also a mess with insurance. Life insurance policies are nullified in the event of suicide. This would have to apply to duels as well, so that people don't purposely get themselves killed just to leave money for their family.
