D'Amico is a reasonably common last name, for instance. (Duh-mico, rather than damm-ih-co.) In general, the apostrophes are there to help place emphasis, if not altogether separate the syllables for you so you don't parse it incorrectly. Lok'tar, for example, would probably not be read as having emphasis on both syllables like the orcs say it.
