...rather than physically? Maybe it is worse if they do start going mentally first rather than physically

I think it's definitely harder on those around them, but tbh they probably have no idea where they are or who anyone/anything is after a certain point. I've seen it and it's pretty messed up.

I think that's also "the point of no return" as well, when the dementia gets real bad. With physical ailments you can always have that hope that they will be able to beat whatever illness they are suffering and get better. Once the mind goes though, there's no coming back from that.
