In high school I would just refer to teachers by their last name. No title.

Same here. I picked up the habit Freshman year in sports (practice started before the school year did) we would use everyone's last names only. To this day I still do it with the teachers I worked with and the teachers my children have/had.

My professors from college and some ol Boy Scouts leaders of mine asked me to call them by first name after I graduated and being an adult now instead of a kid. I just can't bring myself to do it part out of respect for them still and partly because I'm too accustomed calling them by Professor or Mr/Mrs.
