It's not a very huge stretch to assume that he could suffocate her and since it's unlikely an autopsy would be performed, everyone would just think she died of the disease. That's not a problem. And the normal town, that has cults, and a dark history, and is/was a tourist spot. It's very possible to go in the normal version, only to wind up in the mist version after a while.
