I call some woman a cow many times. Doesn't mean I think they are fat. Cow is actually British slang for an annoying, stupid and/or bitchy woman. Sure this is heifer but that just means a young cow as you've already pointed out. It can also refer to someone who eats a lot of food (fat or skinny).
