If I know he goes around sexually harassing women and calls himself "The Don", I would just tell him what I would always be telling him in this scenario: "What did I tell you about harassing women? Count yourself lucky for getting off with just that. Now let's go get that hand fixed up." On the way to the hospital I might ask him what his motivations for behaving this way are, and maybe over there I might see if we can find some sort of therapy for him.
