It all started with Freeza. I didnt start out with any plans to have him transform of course, but midway through I thought it might be cool to make it look like a bluff and then have him transform for real. Probably at that point I also thought of giving him a sleek design in the end. Im in the habit of giving characters progressively more complex and tough-looking forms, then finally making them really sleek. After all, its awful drawing them once they get all complex. (laughs) Complex guys are terrible when you have to draw them for weeks on end Cell was a ton of work, with those darn spots of his. (laughs)

I think there's probably a world in which Raditz took Vegeta's place in the story but seeing as that didn't happen, I think having the both of them would be kinda redundant and is probably the biggest reason Raditz never returned.

Raditz coming back could only really result in one of two things, he turns good, becomes an actual brother to Goku and settles into a more regular lifestyle (which is a repeat of what Vegeta has already done) or he stays bad and just dies again. I guess they could do the former and have him die as redemption anyway but I don't know, I'd hope they could come up with something more interesting after all this time I guess.

Now that's not entirely true. It was to help him push out the manga faster and coloring in the hair took longer. It also allegedly helped animate the anime faster.

Ginyu technically never died before Super. Nappa comes back in the Janemba movie and GT. Cell also comes back in the Janemba movie I think. But those aren't technically canon now.

To be honest, the way I decided upon the Super Saiyan design was for such a... simple reason that itd make you go "eh?". I only use one assistant (note 3), who has always helped me. My assistant always had to spend a lot of time blacking in Goku's hair (note 4), so the biggest reason was to save time, since if Goku became a Super Saiyan, his hair wouldn't have to be blacked in.

GT has always been non canon.

Toriyama only did the designs for some characters in GT as well as the SSJ4 design, he never really had anything to do with the story so it has never been considered canon.

In one of the Budokai(Tenkaichi?) games there was a What If scenario where Raditz gets bumped on the head and forgets who he is. He is lives with the Z Fighters and is a good guy.

The editor Toriyama had during the Buu arc was soft and Toriyama did whatever he wanted.

Presumable, their skillsets would've been wildly different. They only got defeated so quickly because the editor demanded it.
