Strokes don't really discriminate except by age, and everyone gets old. Anyone can be in danger of having a stroke. Plus, you're not directly making fun of stroke victims, you're not mocking the symptoms of a post-stroke person but rather you're saying that something is so bad that it can cause a serious medical issue. It's not really different from saying that something makes you so mad that you're gonna have an aneurysm.
