he's a bit overly anti-car and pro-bicycle (or anything that isn't a car), but the picture you get from it is, yes, NA infrastructure heavily and needlessly favours cars over basically everything else, and in fact NA is woefully behind in this area to an embarrassing degree.

In great part, yes. It's not exactly a secret that the USA insists on prioritizing short-term profit even when it causes significant long-term issues (health care, transport, higher education, the stifling of alternative energy while propping up fossil fuel industry, etc).

Partially yes, partially it's a matter of having mostly designed the cities after the invention of the automobile and therefore taking it into account. Efforts to improve transit or make small changes to alleviate the car-centricity of cities tend to be hamstrung by the rather wealthy oil and auto lobbies.
