I've not had this issue in Elden Ring tbh. I just talk to everyone even if I'm 99% sure they're going to fuck me over. Like I knew Patches trapped his chest, but I still did it. The worst that's going to happen is you die and lose some runes.

if I see an interesting figure, I click on targeting, and if it doesn't target on them, they're okay (for the time being).

there have been quite a few enemies I thought were NPCs, lol.

I was very wary of the NPCs. I kept expecting that every new merchant I found was going to morph into an enemy once I tried speaking to them. The later into the game I got, the more I was convinced the next one would be the one to ambush me.

what NPC, Im playing DS right now.

this topic also reminds me of a time I got drunk and was playing Dark Souls. I could not beat Ornstein so I got super frustrated and killed every NPC. I think I fuck myself so hard that I just ended up quitting the game and its the only From game Ive never beat.

This is why even for some enemies I walk up to them to see if they aggro. Missed a lot of good backstabs that way.
