Beating the game without killing civilians is easy. I was attempting to do a perfect run where I kill NO ONE. I got all the way to Packies first mission with only 4 kills and completed the Playboy X and Dwayne storyline too. Sadly my run ended there as the Packie mission I was up too is almost impossible without kills.
