For instance, in a medieval-based fantasy setting, it's usually not common for the vast majority of people to travel around a lot. Most people just stay in one place their entire life. If someone looks and sounds completely different than other people in a specific region, that can be justified by saying that that person has a travelling profession, like a trader, sailor or mercenary (for example they did this a couple of times in Vikings). If something takes place in a big centrally-located (coastal) city, that's a solid reason for there to be all kinds of ethnicities (which for example they did with the city of Qarth in Game of Thrones). Real-life-based scenarios can be more difficult in this matter, especially if this takes place in a setting where certain ethnicities were very much treated as less than others. One can choose to ignore all of those circumstances, just cast all kinds of ethnicities and simply not mention why that makes sense. This can work, provided it's not blended with elements like discrimination (which was very clearly the case in the last season of Legends of Tomorrow), as such story-telling elements in a historical do clearly conflict with the presence of an ethnically diverse cast.
