Does it happen a lot for you in games? Do games have an easy time getting you to feel this way or do you need a while to observe and think about it? What sort of things make you care? Imagination? Immersion? Not really any different than movies/TV Shows/Plays. If you do not even try to consider the characters as a living entity then anything that happens to them does not matter at all. Horror/suspense films can range from incredibly boring to horrific depending on your ability to immerse yourself into the world and create empathy for the characters like they are living entities. Sometimes if a character feels really relatable I can immediately get immersed in the story and grow attached to them.
